Growth factor gene transfer-induced therapeutic angiogenesis has become a novel approach for the treatment of myocardial ischemia. In order to provide a basis for the clinical application of an adenovirus with hepatocyte growth factor gene (Ad-HGF) in the treatment of myocardial ischemia, we established a minipig model of chronically ischemic myocardium in which an Ameroid constrictor was placed around the left circumflex branch of the coronary artery (LCX). A total of 18 minipigs were randomly divided into 3 groups: a surgery control group, a model group and an Ad-HGF treatment group implanted with Ameroid constrictor. Ad-HGF or the control agent was injected directly into the ischemic myocardium, and an improvement in heart function and blood supply were evaluated. The results showed that myocardial perfusion remarkably improved in the Ad-HGF group compared with that in both the control and model groups. Four weeks after the treatment, the density of newly formed blood vessels was higher and the number of collateral blood vessels was greater in the Ad-HGF group than in the model group. The area of myocardial ischemia reduced evidently and the left ventricular ejection fraction improved significantly in the Ad-HGF group. These results suggest that HGF gene therapy may become a novel approach in the treatment of chronically ischemic myocardium.

Treatment of chronical myocardial ischemia by adenovirus-mediated hepatocyte growth factor gene transfer in minipigs.

Wachstumsfaktor gen induziert durch &Uuml;bertragung therapeutische Angiogenese ist ein neuartiger Ansatz zur Behandlung von myokardialer Ischemia geworden. Um eine Grundlage f&uuml;r die klinische Anwendung der ein Adenovirus mit Hepatozyten-Wachstumsfaktor-gen (Ad-HGF) in der Behandlung von myokardialer Ischemia zu bieten, haben wir ein Minipig-Modell der chronisch isch&auml;mischen Myokard, in dem ein Ameroid Constrictor um den linken circumflex Zweig der Koronararterien (LCX) gebracht wurde. Insgesamt 18 Minipigs wurden nach dem Zufallsprinzip in drei Gruppen aufgeteilt: eine Kontrollgruppe Chirurgie, eine Modellgruppe und ein Ad-HGF-Behandlungsgruppe mit Ameroid Constrictor implantiert. Anzeige-HGF oder der Control Agent direkt in isch&auml;mischen Myokard injiziert wurde und eine Verbesserung der Herz-Funktion und Blut-Versorgung wurden ausgewertet. Die Ergebnisse zeigten, dass myokardiale Perfusion bemerkenswert verbessert in der Ad-HGF-Gruppe gegen&uuml;ber, die in die Kontrolle und die Modell-Gruppen. Vier Wochen nach der Behandlung die Dichte des neu gebildeten Blutgef&auml;&szlig;e h&ouml;her war und die Zahl der Kollateralsch&auml;den Blutgef&auml;&szlig;e in der Ad-HGF-Gruppe als in der Modellgruppe gr&ouml;&szlig;er war. Bereich myokardialer Ischemia offenbar reduziert und die linke ventrikul&auml;re Ejektionsfraktion verbesserte sich deutlich in der Ad-HGF-Gruppe. Diese Ergebnisse legen nahe, dass sich HGF-Gentherapie entwickelt einen neuartigen Ansatz in der Behandlung von chronisch isch&auml;mischen Myokard.

The abnormal decrease or the lack of oxygen supply to cells and tissues is called hypoxia. This condition is commonly seen in various diseases such as rheumatoid arthritis and atherosclerosis, also in solid cancers. Pre-clinical and clinical studies have shown that hypoxic cancers are extremely aggressive, resistant to standard therapies (chemotherapy and radiotherapy), and thus very difficult to eradicate. Hypoxia affects both the tumor and the immune cells via various pathways. This review summarizes the most common effects of hypoxia on immune cells that play a key role in the anti-tumor response, the limitation of current therapies, and the potential solutions that were developed for hypoxic malignancies.

Hypoxic tumors and their effect on immune cells and cancer therapy.

Die abnorme Verminderung oder die mangelnde Sauerstoffzufuhr zu den Zellen und Geweben ist Hypoxie bezeichnet. Diese Bedingung wird h&auml;ufig bei verschiedenen Erkrankungen wie rheumatoide Arthritis und Arteriosklerose, auch in soliden Tumoren angezeigt. Vorklinische und klinische Studien haben gezeigt, dass Hypoxie Krebserkrankungen extrem aggressiv, Therapiestandards (Chemotherapie und Strahlentherapie), abriebfest sind und somit sehr schwer zu beseitigen. Hypoxie wirkt sich der Tumor und die Immunzellen &uuml;ber verschiedene Wege. Diesen Bericht fasst die h&auml;ufigsten Auswirkungen von Hypoxie auf Immunzellen, die eine Schl&uuml;sselrolle in der Anti-Tumor-Antwort, die Beschr&auml;nkung der aktuellen Therapien und die m&ouml;glichen L&ouml;sungen, die f&uuml;r hypoxic Krebserkrankungen entwickelt wurden.
