Humoral circulating inflammatory cytokines such as tumor necrosis factor alpha (TNF-alpha) can impair skeletal muscle contractility. Furthermore, TNF-alpha expression correlates with elevated levels of atrogin-like muscle-specific ubiquitin E3 ligases, which are presumed to mediate muscle protein breakdown and atrophy. However, the casual relationships between MuRF1 and TNF-alpha and their relative contributions to muscle function impairment are not known.

AimsDiastolic dysfunction (DD) was identified as a predictor of adverse prognosis in heart failure with reduced ejection fraction (HFREF). It is, however, unknown if DD is improved by exercise training, which is known to induce reverse remodelling, and if the training effect is attenuated in elderly HFREF patients. We therefore assessed DD in a cohort of referent controls (RCs) and HFREF patients and studied the response of DD to endurance exercise in two age groups (â‰¤55 years and â‰¥65 years).Methods and resultsSixty RC (30 â‰¤ 55 years, mean age 50 Â± 5 years; 30 â‰¥ 65 years, 72 Â± 4 years) and 60 HFREF patients (30 â‰¤ 55 years, 46 Â± 5 years; 30 â‰¥ 65 years, 72 Â± 5 years, EF 28 Â± 5%) were randomized to 4 weeks of supervised endurance training or to a control group. Exercise training was effective in reducing LV isovolumetric relaxation time by 29% in young and by 26% in old HFREF patients (P< 0.05 for both). As assessed by tissue Doppler, septal E' increased by 37% in young and by 39% among old HFREF patients (P< 0.005 for both) resulting in a significant decrease in the E/E' ratio from 13 Â± 1 to 10 Â± 1 in young and 14 Â± 1 to 11 Â± 1 in old HFREF patients (P< 0.05 for both). Serum levels of N-terminal pro brain natriuretic peptide were significantly reduced after endurance training in HFREF patients of all ages.ConclusionIn HFREF, diastolic function is significantly impaired in all age groups. Endurance training is highly effective in improving left ventricular diastolic function in HFREF patients regardless of age.This study is registered at ClinicalTrials.gov (number: NCT00176319).

Age-related effects of exercise training on diastolic function in heart failure with reduced ejection fraction: The Leipzig Exercise Intervention in Chronic Heart Failure and Aging (LEICA) Diastolic Dysfunction Study.

AimsDiastolic Dysfunktion (DD) wurde als ein Pr&auml;diktor f&uuml;r ung&uuml;nstige Prognose in Herzinsuffizienz mit eingeschr&auml;nkter Ejektionsfraktion (HFREF) identifiziert. Es ist jedoch unbekannt wenn DD durch Trainings, verbessert ist die umgekehrte Umbau induzieren bekannt ist, und der Trainingseffekt bei &auml;lteren Patienten in HFREF abgeschw&auml;cht ist. Wir daher DD in einer Kohorte der Referent Steuerelemente (RCs) und HFREF Patienten zu beurteilen und die Reaktion der DD auf Ausdauerbelastungen in zwei Altersgruppen (&le;55 Jahre hinweg &sup3;65) studierte.Methoden und ResultsSixty RC (30 &le; 55 Jahre, Durchschnittsalter 50 &plusmn; 5 Jahre; 30 &ge; 65 Jahre, 72 &plusmn; 4 Jahre) und 60 HFREF Patienten (30 &le; 55 Jahre, 46 &plusmn; 5 Jahre; 30 &ge; 65 Jahre, 72 &plusmn; 5 Jahre EF 28 &plusmn; 5 %) wurden randomisiert um 4 Wochen betreutes Ausdauertraining oder eine Kontrollgruppe. Trainings war wirksam bei der Verringerung der LV Isovolumetric Relaxationszeit von 29 % bei jungen und um 26 % im alten HFREF-Patienten (P &lt; 0,05 f&uuml;r beide). Die Gewebe Doppler, Septumdefekt E bewerten &#39; stieg um 37 % bei jungen und um 39 % unter alten HFREF Patienten (P &lt; 0,005 f&uuml;r beide) was zu einem deutlichen R&uuml;ckgang der E/E&#39; Verh&auml;ltnis von 13 &plusmn; 1 bis 10 &plusmn; 1 bei jungen und 14 &plusmn; 1 bis 11 &plusmn; 1 bei alten HFREF Patienten (P &lt; 0,05 f&uuml;r beide). Serumspiegel von N-Terminal pro-Brain natriuretisches Peptid wurden nach Ausdauertraining bei HFREF Patienten jeden Alters deutlich reduziert.ConclusionIn HFREF, Diastolische Funktion ist in allen Altersgruppen wesentlich beeintr&auml;chtigt. Ausdauertraining ist sehr effektiv bei der Verbesserung der linken ventrikul&auml;ren diastolischen Funktion bei HFREF Patienten unabh&auml;ngig vom Alter.Diese Studie ist registriert bei ClinicalTrials.gov (Nummer: NCT00176319).
