Tibial Cortex Transverse Transport (TTT) represents an innovative surgical method for treating lower extremity diabetic foot ulcers (DFUs), yet its underlying mechanisms remain elusive. Establishing an animal model that closely mirrors clinical scenarios is both critical and novel for elucidating the mechanisms of TTT.

The clinical management of patients with chronic limb-threatening ischemia (CLTI) faces great challenges. Enhancing wound healing and limb preservation rates in this cohort is a critical objective. This study investigates the effectiveness of combining tibial cortex transverse transport (TTT) and endovascular therapy (EVT) for the treatment of patients with severe CLTI. We aim to evaluate the therapeutic results of this combined approach on the specified patient group.

Inflammatory cytokines are key indicators affecting the development of ulcers of lower limb. The causal role of inflammatory cytokines in ulcers of lower limb and whether this can be mediated by metabolites remain unknown. We conducted a two-step, two-sample Mendelian randomization (MR) study to investigate the causal effect of inflammatory cytokines on ulcers of lower limb and the mediating role of metabolites in the association between inflammatory cytokines and ulcers of lower limb. MR analysis identified seven inflammatory cytokines (CCL19, IL-12β, MCP4, MIP1a, SCF, sirtuin2, and TNFSF9) that promote ulcers of lower limb. Additionally, 56 metabolites were found to be associated with ulcers of lower limb. Mediation MR indicated that the causal effect of sirtuin2 on ulcers of lower limb (total effect Inverse Variance Weighted [IVW]: odds ratio [OR] = 1.162, 95% confidence interval [CI] [1.051, 1.285], p = 0.003) is largely mediated by 4-hydroxyphenylacetate (0.0185, 95% CI [-0.00278, 0.0397], accounting for 11.1% of the total effect).

This study aims to compare the effects of tibial cortex transverse transport (TTT) and platelet-rich plasma (PRP) on the healing of severe diabetic foot ulcers, evaluate the clinical efficacy of TTT, and explore its potential impact on lower limb circulation.

Bacterial infection can delay wound healing, while drug resistance further complicates the treatment of wound infection. Phototherapy, including photothermal therapy (PTT) and photodynamic therapy (PDT), is a non/mini-invasive and efficient antibacterial strategy that rarely induces bacterial resistance. This treatment relies on specific wavelengths of light to activate photothermal agents (PTAs) or photosensitizers, killing bacteria by generating local heats or reactive oxygen species (ROS), respectively. However, the light for traditional PTT/PDT mainly falls in the visible and near-infrared I light (Vis/NIR-I light, 400-900 nm) regions, which significantly limits further clinical translations due to its low tissue permeability. The near-infrared II (NIR-II,1000-1700 nm) light is increasingly utilized in antibacterial PTT/PDT to improve tissue penetration and ameliorate the immune microenvironment of deeper wounds. Meanwhile, NIR-II light offers a higher maximum permissible exposure (MPE) for PTT/PDT in treating wound infections, thereby facilitating the security, in comparison to Vis/NIR-I light. This review highlights recent advancements in NIR-II PTT/PDT for drug-resistant wound infections, focusing on mechanisms, therapeutic outcomes, challenges, and prospects.
