Neutrophilic inflammation causes lung damage in cystic fibrosis (CF). Recent data from animal models suggest that the migration of blood monocytes into the airway supports neutrophil-mediated tissue injury. CF may therefore be associated with increased airway levels of chemoattractants for pro-inflammatory monocytes. In this study, we sought to assess the levels of monocyte chemoattractants CCL2 and CX3CL1 in the blood and airways of patients with CF, and expression of their respective receptors CCR2 and CX3CR1 on blood monocytes.

Monozyt Chemoattractant Chemokine bei zystischer Fibrose.

Macrophages in the airways form an important element of immune defense and inflammation. We analyzed induced sputum from airways of patients with CF for the types of macrophages present, their receptor expression, and phagocytic function. In samples from patients and age-matched controls, macrophages were analyzed by multicolor flow cytometry, scavenger receptor expression was studied at the protein and mRNA level, and receptor function was investigated using fluorescent particles. In adult patients with CF, we discovered a pronounced expansion of the small CD14+ DR+ CD68weak+ macrophages to 73 +/- 18% compared with 16 +/- 8% in healthy controls. Expression of the MARCO and CD206 (mannose receptor) was strongly reduced at the mRNA and protein level in sputum macrophages. Antibody-blocking studies showed that MARCO mediates phagocytosis of unopsonized particles. In line with reduced MARCO expression, sputum macrophages in CF showed a deficient uptake of particles (23+/-9% of cells) compared with healthy controls (71+/-15%). The deficiency of MARCO expression in the predominant small sputum macrophages in CF may lead to impaired clearance of inhaled particles with increased inflammation and damage to the CF lung.

Conjugate pneumococcal vaccines offer suboptimal protection against mucosal infections and are restricted in serotype and geographical coverage. New protein-based vaccines using conserved pneumococcal antigens and better mucosal adjuvant technology are urgently needed. Interleukin-12 (IL-12) has shown efficacy as a pneumococcal protein vaccine adjuvant in murine models of pneumococcal infection. Systemic administration of recombinant human (rh) IL-12 to humans, however, has been associated with adverse clinical and laboratory side effects. Inhaled forms of IL-12 have improved the safety profiles in humans, as suggested by animal models. Here we evaluated rhIL-12 as an adjuvant on ex vivo human BAL cells when stimulated with pneumococcal whole cells. We show that co-incubation of ex vivo human BAL cells with pneumococcal whole cell antigen (WCA) and a low dose of rhIL-12 (2 ng) can elevate TNF production compared to treatment with WCA (p=0.06) or rhIL-12 (p=0.03) alone. The production of IFNÎ³ was also increased but not in an antigen specific manner, suggesting perhaps a predominant Th(1) response. Our data suggest that 100-200-fold lower doses of inhaled rhIL-12 than those previously tested for systemic use may be adequate in a phase 1 study and commend further evaluation of rhIL-12 as a potential mucosal adjuvant in human vaccine studies.

Wir zeigen, dass Co-incubation der ex Vivo Humanzellen BAL mit Pneumokokken ganze Zelle Antigen (WCA) und eine niedrige Dosis von RhIL-12 (2 ng) k&ouml;nnen im Vergleich zur Behandlung mit WCA TNF-Produktion erh&ouml;hen (p = 0,06) oder RhIL-12 (p = 0,03) allein. Recently, an established "small macrophage" phenotype has been observed in the sputum of patients with CF and COPD. However, little is known about the prevalence of this phenotype in the airways of young children. Since respiratory inflammation begins early in CF, we hypothesised that these small macrophages would be increased in paediatric CF bronchoalveolar lavage (BAL).

Infectious challenge of the human nasal mucosa elicits immune responses that determine the fate of the host-bacterial interaction; leading either to clearance, colonisation and/or disease. Persistent antigenic exposure from pneumococcal colonisation can induce both humoral and cellular defences that are protective against carriage and disease. We challenged healthy adults intra-nasally with live 23F or 6B Streptococcus pneumoniae in two sequential cohorts and collected nasal wash, bronchoalveolar lavage (BAL) and blood before and 6 weeks after challenge. We hypothesised that both cohorts would successfully become colonised but this did not occur except for one volunteer. The effect of bacterial challenge without colonisation in healthy adults has not been previously assessed. We measured the antigen-specific humoral and cellular immune responses in challenged but not colonised volunteers by ELISA and Flow Cytometry. Antigen-specific responses were seen in each compartment both before and after bacterial challenge for both cohorts. Antigen-specific IgG and IgA levels were significantly elevated in nasal wash 6 weeks after challenge compared to baseline. Immunoglobulin responses to pneumococci were directed towards various protein targets but not capsular polysaccharide. 23F but not 6B challenge elevated IgG anti-PspA in BAL. Serum immunoglobulins did not increase in response to challenge. In neither challenge cohort was there any alteration in the frequencies of TNF, IL-17 or IFNÎ³ producing CD4 T cells before or after challenge in BAL or blood. We show that simple, low dose mucosal exposure with pneumococci may immunise mucosal surfaces by augmenting anti-protein immunoglobulin responses; but not capsular or cellular responses. We hypothesise that mucosal exposure alone may not replicate the systemic immunising effect of experimental or natural carriage in humans.

Wir gesunde Erwachsene intra-nasally mit live 23F oder 6B Streptococcus Pneumoniae in zwei sequentiellen Kohorten in Frage gestellt und sammelte nasal-Waschanlagen, Bronchoalveol&auml;re Lavage (BAL) und Blut vor und 6 Wochen nach Herausforderung. Wir gehen, dass beide Kohorten erfolgreich kolonisiert werden w&uuml;rde, aber dies kam nicht bis auf einen freiwilligen. Antigen-spezifische IgG und IgA Niveaus wurden deutlich in nasale W&auml;sche erhoben 6 Wochen nachdem Herausforderung Ausgangswerten verglichen. Immunglobulin Antworten zu Pneumokokken wurden an verschiedenen Protein Ziele aber nicht Kapselspannring Polysaccharid gerichtet. 23F aber nicht 6B Herausforderung erh&ouml;hten IgG Anti-PspA im BAL. Serum Immunglobuline als Reaktion auf die Herausforderung nicht gestiegen. Kohorte war weder Herausforderung es &Auml;nderungen in den Frequenzen der TNF, IL-17 oder IFN&gamma; CD4 T-Zellen produzierend, vor oder nach der Herausforderung in BAL oder Blut. Wir zeigen, dass einfache, niedrige Dosis Schleimhaut-Exposition mit Pneumokokken kann Schleimh&auml;ute durch eine Verst&auml;rkung der anti-protein Immunglobulin Antworten auszuklammern; aber nicht Kapselspannring oder zellul&auml;re Antworten. Wir Kriegsfolgekosten, die Schleimhaut Exposition allein kann nicht replizieren, die systemische Immunisierung Wirkung der experimentellen oder nat&uuml;rliche Bef&ouml;rderung beim Menschen.

RATIONALE: The immunological and protective role of pneumococcal carriage in healthy adults is not known but high rates of disease and death in elderly are associated with low carriage prevalence. OBJECTIVES: We employed an experimental human pneumococcal carriage model to investigate the immunizing effect of a single carriage episode. METHODS: Seventy healthy adults were challenged and of those with carriage 10 were re-challenged intranasally with live 6B Streptococcus pneumoniae up to 11 months after clearance of first carriage episode. Serum and nasal wash antibody response were measured before and after each challenge. MEASUREMENTS AND MAIN RESULTS: 29 subjects were experimentally colonized. No subjects were colonized by experimental re-challenge demonstrating the protective effect of initial carriage against subsequent infection. Carriage increased both mucosal and serum IgG levels to pneumococcal proteins and polysaccharide, resulting in a fourfold increase in opsonophagocytic activity. Importantly, passive transfer of post-carriage sera from colonized subjects conferred 70% protection against lethal challenge by a heterologous strain in a murine model of invasive pneumococcal pneumonia. These levels were significantly higher than the protection conferred by either pre-carriage sera (30%) or saline (10%). CONCLUSIONS: Experimental human carriage resulted in mucosal and systemic immunological responses that conferred protection against re-colonization and invasive pneumococcal disease. These data suggest that mucosal pneumococcal vaccination strategies maybe important for vulnerable patient groups, particularly the elderly, who do not sustain carriage.

Controlled Human Infection and Re-Challenge with Streptococcus Pneumoniae Reveals the Protective Efficacy of Carriage in Healthy Adults.

Begr&uuml;ndung: Die immunologische und sch&uuml;tzende Rolle Pneumokokken Bef&ouml;rderungsbedingungen bei gesunden Erwachsenen ist nicht bekannt, aber hohe Raten von Krankheit und Tod im alten niedrigen Bef&ouml;rderung Pr&auml;valenz zugeordnet sind. Ziele: Wir besch&auml;ftigt eine experimentelle menschlichen Pneumokokken Bef&ouml;rderung-Modell den immunisierenden Effekt einer Einzelwagen-Episode zu untersuchen. Methoden: Siebzig gesunde Erwachsene wurden herausgefordert und derjenigen mit Kutsche 10 wurden re-challenged intranasally mit live-6B Streptococcus Pneumoniae bis bis 11 Monate nach Freigabe der ersten Bef&ouml;rderung Folge. Serum und nasal-Waschanlagen-Antik&ouml;rper-Reaktion wurden vor und nach jeder Herausforderung gemessen. Messungen und Hauptergebnisse: 29 Themen wurden experimentell kolonisiert. Keine Themen wurden durch experimentelle Re-challenge demonstrieren die Schutzwirkung des ersten Bef&ouml;rderung gegen sp&auml;tere Infektionen kolonisiert. Bef&ouml;rderung erh&ouml;ht sowohl Schleimhaut- und Serum IgG Ebenen Pneumokokken Proteine und Polysaccharid, was eine Vervierfachung in Opsonophagocytic Aktivit&auml;t. Wichtig ist, verlieh passiven &Uuml;bertragung der post-carriage Seren von kolonisierten Themen 70 % Schutz gegen t&ouml;dliche Herausforderung durch eine heterologe Belastung in einem murinen Modell von invasiven Pneumokokken Lungenentz&uuml;ndung. Diese Ebenen waren deutlich h&ouml;her als der Schutz durch Nachlauf Seren (30 %) oder Kochsalzl&ouml;sung (10 %). Schlussfolgerungen: Experimentelle menschlichen Bef&ouml;rderung f&uuml;hrte Schleimhaut- und systemische Immunreaktionen, die Schutz gegen Rekolonialisierung und invasive Pneumokokken-Krankheit gew&auml;hrt. Diese Daten legen nahe, dass Schleimhaut Pneumokokken Impfstrategien vielleicht wichtig f&uuml;r gef&auml;hrdete Patienten Gruppen, besonders die &auml;lteren Menschen, die Bef&ouml;rderung nicht aufrechterhalten werden.
